Access to transportation, baby supplies, stable housing, language support, and trusted community resources can all play a role in a family\u2019s health and well-being.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><strong>Community Health Workers are often the people helping families navigate those challenges. They connect patients with resources, help remove barriers to care, and serve as a bridge between health care systems and communities.\u00a0<\/strong><\/p>\n<p><span data-contrast=\"auto\">But Community Health Workers also need connections \u2014 trusted partners and reliable networks that help them find resources for the families they serve.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">That&#8217;s\u00a0where <\/span><strong>Help Me Grow \u12cb\u123d\u1295\u130d\u1270\u1295<\/strong><span data-contrast=\"auto\">,\u00a0<a href=\"http:\/\/withinreachwa.org\" target=\"_blank\" rel=\"noopener\"><span style=\"text-decoration: underline;\"><span style=\"color: #ff671d;\"><strong>WithinReach<\/strong><\/span><\/span><\/a>&#8216;s\u00a0flagship program, comes in. I work\u00a0in what\u2019s called\u00a0a specialty clinic, specifically women\u2019s health.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The way I describe my role is\u00a0that I\u00a0help pair patients with community resources. Community Health Workers came out of research showing how social needs \u2014 or social drivers of health \u2014 affect a patient\u2019s overall health and well-being.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">The idea is that providers can focus on the medical side of care while I help with the social side. I work alongside providers and midwives to help address\u00a0barriers\u00a0that may prevent someone from getting the care they need.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p aria-level=\"3\"><span style=\"color: #571653;\"><b>Kate: You work primarily in women\u2019s health. Can you tell us more about the communities you serve?<\/b>\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">We have physicians, midwives, and gynecologists, and I support our West Seattle, Des Moines, and Burien Women\u2019s Health clinics.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">We also serve patients who may live farther away but work in this area, or patients who transfer their care to us. We serve a large migrant population and many patients who do not speak English.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Occasionally, we also work with patients experiencing substance use, and I work closely with a social worker at the hospital\u2019s birthing center to support those families.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><strong><span style=\"color: #571653;\">\u00a0Kate: What are some of the most common needs or challenges you see families navigating?\u00a0<\/span><\/strong><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">Support is a huge one. Supplies are another, especially with the cost of everything today. Families with more than one child may need help finding things like baby supplies, whether\u00a0that\u2019s\u00a0through Pregnancy Aid, a church, or other small community programs.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Transportation is another huge need. We want patients to be able to get to their OB appointments and receive the care they need.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">I\u00a0predominantly work\u00a0with pregnant people and expecting mothers, but occasionally I also support gynecology patients who may be navigating challenges like billing systems or other barriers.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Every family is different. Not every community organization has the funding or language capacity to support every patient. When I can\u2019t find what I\u2019m looking for in the community, I think: \u201cHelp Me Grow.\u201d<\/span><\/p>\n<p><span data-contrast=\"auto\">You have a much bigger view of\u00a0what\u2019s\u00a0out there than I do. Because\u00a0you\u2019re\u00a0statewide, you have access to resources that I may not even know exist.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p aria-level=\"3\"><span style=\"color: #571653;\"><b>Kate: What does Help Me Grow Washington support look like for your families?<\/b>\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">One patient who really stands out was in the Bremerton and Port Orchard area. This patient was too far along in her pregnancy to qualify for one program, and another program\u00a0wasn\u2019t\u00a0available in her area.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">There\u00a0wasn\u2019t\u00a0a strong community resource center specifically for pregnant women, so I\u00a0submitted\u00a0a referral to Help Me Grow Washington.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Around that same time, I learned about the closed-loop referral feature. When I first started making referrals, that\u00a0didn\u2019t\u00a0exist, so I always wondered, \u201cDid they connect with the patient? What happened?\u201d<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">It was so helpful to see that Help Me Grow had connected with her. I could see that she had received a gift card, and I learned about resources and partners I\u00a0didn\u2019t\u00a0even know existed.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">That helped the patient, but it also helped me.\u00a0I\u2019m\u00a0learning about different communities, different resources, and different partners across the state.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p aria-level=\"3\"><span style=\"color: #571653;\"><b>Kate: How has Help Me Grow supported families who may face\u00a0additional\u00a0barriers\u00a0accessing\u00a0resources?<\/b>\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">It\u2019s\u00a0been especially helpful for many of my patients who are immigrants. Some families are afraid of government programs, worry they\u00a0won\u2019t\u00a0qualify, or are concerned about what will happen if they share their information.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Help Me Grow has been a safe resource for many of those patients.\u00a0That\u2019s\u00a0been huge because\u00a0there\u2019s\u00a0a lot of fear, especially if you\u00a0don\u2019t\u00a0speak English,\u00a0you\u2019re\u00a0new to this country, and\u00a0you\u2019re\u00a0already dealing with so many other challenges.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">I\u2019ve\u00a0had patients decline resources because\u00a0they\u2019re\u00a0afraid that if they accept help, they\u00a0won\u2019t\u00a0be able to get help somewhere else, or that there will be consequences for asking for support.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p aria-level=\"3\"><span style=\"color: #571653;\"><b>Kate: You also support families experiencing substance use during pregnancy and postpartum. What challenges do you see when connecting these families to support?<\/b>\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">Substance use is a huge one because there\u00a0aren\u2019t\u00a0always a lot of resources available, especially for pregnant people.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">There are programs out there, but access can depend on where someone lives and what services are available in their community. Sometimes families need more support navigating\u00a0what\u2019s\u00a0available and figuring out the next step.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Providers are there to\u00a0monitor\u00a0the medical side of pregnancy, but they\u00a0aren\u2019t\u00a0always able to walk alongside a family through every resource connection.\u00a0That\u2019s\u00a0where Community Health Workers and programs like Help Me Grow can be\u00a0really important.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Sometimes a patient just needs a little more hand-holding, and\u00a0Help Me Grow provides\u00a0that connection.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p aria-level=\"3\"><span style=\"color: #571653;\"><b>Kate: After working with families and using Help Me Grow Washington as a resource, what stands out most to you about this partnership?<\/b>\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">I\u2019m\u00a0happy you\u00a0all are\u00a0there.\u00a0I\u2019m\u00a0glad that Help Me Grow is a program focused on continuous improvement.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">It shows that the heart is really there.\u00a0It\u2019s\u00a0not just a program \u2014\u00a0it\u2019s\u00a0a program that wants to be effective and reach people.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">You can tell there\u2019s passion behind the work.\u00a0It\u2019s\u00a0not just a job. I can tell from interacting with you that you want to do well and that\u00a0you\u2019re\u00a0really trying to make sure families are supported.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p aria-level=\"3\"><span style=\"color: #571653;\"><b>Kate: What opportunities do you see for more Community Health Workers and other partners to connect with Help Me Grow Washington?<\/b>\u00a0<\/span><\/p>\n<p><b><span data-contrast=\"auto\">Alicia:\u00a0<\/span><\/b><span data-contrast=\"auto\">I wish more people knew about Help Me Grow. It saves so many phone calls and so much time waiting for people to return phone calls.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">I think schools would really\u00a0benefit\u00a0from\u00a0knowing\u00a0about this resource. Sometimes families are navigating financial challenges, clothing needs, transportation issues, or housing instability, and school counselors are trying to figure out what resources are available.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">I also think about Community Health Workers in primary care and family medicine settings. Originally, our Community Health Worker program started in primary care, and\u00a0I\u2019ve\u00a0been sharing with my team that Help Me Grow Washington can support families across the state.<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><\/p>\n<p><span data-contrast=\"auto\">Local partnerships are important, but sometimes there are limitations.
