Refer a Family

Health and service providers can refer children, families, and caregivers they work with to Help Me Grow Washington using our online referral form. With consent from the caregiver, you’ll share some information with our Family Resource Navigators, who will reach out and connect with them to talk about helpful resources and services. No income or eligibility requirements exist for using Help Me Grow Washington services, so no person will be turned away from our support.

What happens when I make a referral?

Refer

When you’ve identified a child or a caregiver who could benefit from Help Me Grow, you or someone on your team will fill out our secure, online Help Me Grow Referral Form.

Listen

When one of our Family Resource Navigators responds to the referral, they will ask the caregiver some questions and listen to their concerns. This is to ensure we find the appropriate resources and services to meet the needs of the child and family.

Connect

In some cases, the Family Resource Navigators will offer information and education; in others they will offer direct referrals or application assistance or enrollment support.

Follow-Up

Depending on the level of support needed, Family Resource Navigators will follow up with a family to see if there are any barriers to getting support that they can help address.
